－197－EL19Department of Internal Medicine, National University Hospital, SingaporeJason PhuaAsia is the world’s largest and most populous continent, which naturally means that it is the continent with the most number ofcritically ill patients at any one time. Sadly though, because the research infrastructure is relatively under-developed in mostAsian countries, much less is known about the state of intensive care medicine in Asia than in North America, Europe, andAustralia and New Zealand. A new dawn has however arrived, with research being slowly introduced into the DNA of moreand more Asian intensive care units（ICUs）. Today, we are also seeing more research collaborations across Asian ICUs, cities,and countries. It is hoped that these collaborations and the information gained from the studies performed will progressivelyimprove the practice of intensive care medicine and the outcomes of critically ill patients in Asia.教育講演 19 2月12日（金）　14：40～15：30　第12会場Multicentre research in Asian intensive care ?a new dawnEL20国際医療福祉大学熱海病院　神経内科　脳卒中・神経センター永山 正雄救急医療の場において脳神経救急の占める割合は広義には40～50%に達するものと推定されている。さらに狭義の脳神経疾患は勿論、一般に重症度が高ければ高い程、脳神経系合併症の頻度は高く、米国のGeneral ICU入室例の検討では、脳神経疾患以外によるICU 入室例の12% で脳神経系合併症がみられている。一方、低体温（体温管理）療法や脳機能モニタリングさえ行っていれば脳神経救急・集中治療（ニューロクリティカルケア）を行っているとの考えは大きな誤りである。的確な脳神経救急・集中治療の実践のためには、最低限の知識と経験に加えて、的確かつ簡潔な医療面接（問診）と臨床解剖を踏まえた身体診察、リアルタイムの鑑別診断とdecision-making が求められる。実際、米国バージニア大学救急部門での検討（2002 年）では、急性意識障害の診断に有用であった診断のモダリティーは現病歴51%、既往歴43%, 診察41%、治療への反応39%、画像 16%の順であった。また脳神経系の病気の診断の約80%は医療面接（問診）と診察のみから可能ともいわれる。また救急・集中治療を要する場面では、包括性、正確性を追求しすぎること無く、T.P.O.（Time、Place、Occasion）をわきまえて時間との勝負を両立させる必要がある。本教育講演では、臨床研修医、救急・集中治療後期研修医、若手集中治療医、集中治療に従事する看護師ほかのコ・メディカル諸氏の診療に役立つ内容で、脳神経救急・集中治療に必要なベッドサイドにおける脳神経所見の診かたを、多数の動画も交えて限りなくわかり易くご説明したい。教育講演 20 2月12日（金）　14：40～15：30　第13会場救急・集中治療に必要な脳神経所見の診かた
